Build and share a reusable form

Create a public form, configure its design and response behavior, and share or embed it.

Reusable forms collect the same type of information from many people through a stable public link. Build the questions once, control what happens after submission, and share the form directly or embed it on your website.

Create a form

Select New form, then begin blank, choose the template library, or build a first draft with Moxie Assistant. Name the form clearly and add the elements respondents need. Mark only truly necessary questions as required.

Edit the form canvas and its design or behavior in the same workspace.

Design the respondent experience

Use Design to set typography, field appearance, button styling, and layout. Use Preview to review the public experience before distributing the link.

Configure what happens after submission

Under Behavior, set the confirmation message, redirect URL, and confirmation email template. The Create records section can add a submission to the pipeline or create a ticket from a public form.

Activate and share

An active form is available at its public link, and saved edits publish immediately. Select Share to copy the link, download a QR code, copy an embed snippet, or send the form by email.

Organize the library

From the Forms list, switch between active, inactive, and all forms. Use folders directly in the list to keep a larger form library organized, and use card or table view based on how you prefer to scan it.
