Build and share a reusable form

Create a public form, configure its design and response behavior, and share or embed it.

Reusable forms collect the same type of information from many people through a stable public link. Build the questions once, control what happens after submission, and share the form directly or embed it on your website.

Create a form

Select New form, then begin blank, choose the template library, or build a first draft with Moxie Assistant. Name the form clearly and add the elements respondents need. Mark only truly necessary questions as required.

Edit the form canvas and its design or behavior in the same workspace.

Design the respondent experience

Use Design to choose a published workspace Brand Kit or set this form's typography, field appearance, button styling, and layout. A Brand Kit controls the visual settings while assigned; the form keeps its saved styles and custom CSS so they return if you remove the kit. Button wording, icons, layout, questions, and behavior remain editable. Use Preview to review the same branded experience respondents see on the public site or in the client portal.

Colors you apply to text blocks with the rich-text editor are preserved in preview, public and embedded forms, and client portal forms, including when a Brand Kit is assigned. Check that your text is easy to read against the form background.

When a form allows multiple file uploads, respondents can reopen the file picker to add more files without replacing earlier selections. Each selected file appears as a chip and can be removed individually before submission. After submitting, a progress spinner remains visible while the form and any file attachments upload.

Configure what happens after submission

Under Behavior, set the confirmation message, redirect URL, and confirmation email template. The Create records section can add a submission to the pipeline or create a ticket from a public form.

Activate and share

An active form is available at its public link, and saved edits publish immediately. Select Share to copy the link, download a QR code, copy an embed snippet, or send the form by email.

Organize the library

From the Forms list, switch between active, inactive, and all forms. Use folders directly in the list to keep a larger form library organized, and use card or table view based on how you prefer to scan it.

Replace a legacy V1 form

V1 forms appear in the Forms list for reference but can no longer be opened, edited, or shared. Select Create V2 copy to create a new editable form from the supported content and settings in the V1 form. Moxie renames the old form with an _v1 suffix and gives the new V2 form its original name and link, so existing embeds and shared links open the new form. Review the converted form before using it, then select Delete on the V1 form if you no longer need the old copy.
